Finasteride causes shrinkage of the prostate by epithelial atrophy, however takes up to 12 months to work. It reduces BPH progression in terms of symptoms, risk of urinary retention and the need for surgery. In this study of 3040 men randomized to finasteride 5mg daily or placebo, the key results were that a PSA level > 1.4 ng/ml and/or a prostate volume in the higher tertiles (anywhere from 14-150 mL) were strong predictors of men who will have the most symptom score and flow rate improvement.  Men with smaller glands
